Inclusion
- member of participating primary care practice network
- able to provide informed consent
- able to complete data assessment tools in English
Exclusion
- diagnosis of CHD, stroke, diabetes, breast cancer, ovarian cancer, or colorectal cancer
- currently pregnant
- evidence of cognitive impairment
