Eligibility Criteria

Inclusion

  • participation in a Medicaid managed care plan
  • resident of identified zip codes in Brooklyn, NY
  • frequent hospitalizations and low frequency of primary doctor visits

Exclusion

  • residents outside the catchment area
  • patients not being managed in a Medicaid managed care plan
