Inclusion

  • Participated in the ALTA, EDEN, OMEGA, or SAILS study
  • Survived until hospital discharge

Exclusion

  • Does not understand or speak English
  • Has no fixed address
  • Pre-existing cognitive impairment that prevents completion of the study assessments
