Eligibility Criteria
Inclusion
- Discharged from hospital with CHD
- Age 75 years or less
- Living in hospital catchment area
- Able to understand Swedish
- Willing to accept randomized group
Exclusion
- Psychiatric disease
- Participated in similar study previously
