Eligibility Criteria

Inclusion

  • Patients with type 2 diabetes from participating practices across NHS Leicester, Warwickshire and Coventry.

Exclusion

  • Patients unable to give informed consent,
  • Terminal cancer, OR
  • Pregnant.
