Eligibility Criteria
Inclusion
- Uncontrolled diabetes patients (defined as HbA1c 6.0 or greater).
- 25 years or older
- English or Spanish speaking
Exclusion
- Uncorrectable hearing or visual impairment
- Moderate to severe cognitive deficits
