Eligibility Criteria
Inclusion
- Age 65 years or older
- A1c greater than or equal to 9.0
Exclusion
- Not expected to live for the duration of the study
- Unwillingness to participate and attend pharmacist visits
- Unwillingness to participate the questionnaires(MMSE, GDS)
