Eligibility Criteria

Inclusion

  • Patient who is clinically diagnosed with type 2 diabetes mellitus.
  • Patient with HbA1C value \> 7% at least within the past two months.
  • Patient who can speak and understand English.
  • Patient of age 21 to 75 years old.
  • Patient with either a smartphone or i-pad/ tablet.

Exclusion

  • Pregnant patient.
  • Patient diagnosed to have a terminal illness.
  • Patient with cognitive impairment.
  • Patient with end-stage renal failure (ESRF).
