Eligibility Criteria

Inclusion

  • Patients must be 18 years or older
  • Must be able to read
  • Must speak English
  • Capable of providing informed consent
  • Must be receiving continuity care from and enrolled clinician

Exclusion

  • Currently hospitalized
  • Awaiting heart transplant
