Eligibility Criteria
Inclusion
- Admitted to the medicine service of a large, urban hospital
- Taking 5 or more chronic medications
Exclusion
- Plan to discharge patient to hospice, rehabilitation, or skilled nursing facility (i.e., not to the community)
