Eligibility Criteria
Inclusion
- lived in the community at least three months
- over 65 years old
- fluently communicated in Mandarin or Taiwanese
- screened by the High-need Community Older Adults Screening Scale (HCOASS) as high-need older adults
Exclusion
- diagnosed as dementia
