Eligibility Criteria
Inclusion
- Adult patient 18 years of age
- acute ischemic stroke
Exclusion
- hemorrhagic stroke
- prior living status was at a long-term care facility.
- Transient ischemic attack
- being discharged to home, home with rehab or long term care
- not able to provide consent.
