Eligibility Criteria
Inclusion
- Patients who are 18 years or older with Heart Failure diagnosis as defined by internationally recognized guidelines.
- Current residents of the Hashemite Kingdom of Jordan
Exclusion
- Patients who refuse or unable to sign a consent form.
- Patient inability to follow up with provider
