Eligibility Criteria
Inclusion
- Individuals aged 65 and older will be considered.
- Adults who may or may not have cognitive impairments.
- Practices affiliated with Northwestern Medicine and Access Community Health Network.
Exclusion
- 1\. Severe, uncorrectable vision or hearing impairment
