Inclusion
- Black women (self-report)
- Adult ages 30-79 years
- Stage 2 or greater Cardiovascular-Kidney-Metabolic Syndrome
- English speaking
- Lives in Metropolitan Birmingham, AL area.
Exclusion
- \- Healthcare provider-imposed physical activity limitations.
