Eligibility Criteria

Inclusion

  • Male or Female ≥ 60 years old
  • Documentation of HFpEF according to clinical practice guidelines (signs \& symptoms of HF AND ejection fraction ≥50%)

Exclusion

  • Previously referred to hospice care
  • Prior completion of advance directives
  • Moderate-severe dementia or psychiatric disorder precluding informed consent.
