Eligibility Criteria
Inclusion
- Male or Female ≥ 60 years old
- Documentation of HFpEF according to clinical practice guidelines (signs \& symptoms of HF AND ejection fraction ≥50%)
Exclusion
- Previously referred to hospice care
- Prior completion of advance directives
- Moderate-severe dementia or psychiatric disorder precluding informed consent.
